Make this information work for you
Notice the parts that fit your own question. Keep a short record of changes, bring your current medication list and use the questions below to prepare for a conversation. This guide cannot diagnose a condition or select a treatment for you.
What erectile dysfunction means
Recognizing a repeated change
Erectile dysfunction describes difficulty getting or keeping an erection firm enough for sexual activity. An occasional difficulty does not establish a diagnosis, but a repeated change is worth discussing. The pattern matters: when it began, whether it happens every time, and whether desire, ejaculation or pain have changed as well. These are related parts of sexual health, but they are not interchangeable problems.
Starting the conversation
You do not need to work out the cause before asking for help. A short, factual description is enough to begin the conversation. For example, explain what has changed over the past few weeks or months and how it affects you. An online list of symptoms cannot replace an assessment of your own circumstances.
Physical and psychological causes can overlap
Blood vessel disease, diabetes, nerve problems and some hormonal conditions can contribute to ED. Certain prescription medicines can also affect sexual function. Stress, depression, anxiety and relationship difficulties may play a part, sometimes alongside a physical cause. A symptom that varies between situations is useful information, but it does not prove that the problem is only psychological.
Share a complete medication and supplement list, including products bought online. Do not stop a prescribed medicine because you suspect it affects erections; a clinician can assess alternatives while protecting the condition it treats. Treating the underlying problem may be as important as treating the erection difficulty itself.
What happens during an ED assessment
History, examination and selected tests
An assessment generally includes a medical and sexual history and a physical examination. Depending on the findings, a clinician may arrange blood tests or other investigations. They may ask about morning erections, previous pelvic surgery, alcohol use, smoking, sleep and emotional wellbeing. Tests should answer a specific clinical question rather than simply reproduce a standard shopping list.
What to bring to the appointment
Bring the names and strengths of medicines you use, and describe previous ED treatments accurately. Mention whether you tried a medicine once or repeatedly, followed its instructions, experienced side effects, or found the timing difficult. This helps distinguish a treatment that was unsuitable from one that needs a different discussion about how it is used.

Which oral ED medicines are available?
How PDE5 inhibitors fit into treatment
Sildenafil, tadalafil, vardenafil and avanafil belong to the PDE5 inhibitor group. They support the blood-flow response involved in an erection and require sexual stimulation; taking a tablet is not a guarantee of an erection. These medicines do not directly treat every cause of low desire or relationship distress.
Choosing an option around your circumstances
The useful comparison is not simply which medicine is strongest. Discuss how long you would like flexibility around sexual activity, how often you expect to use treatment, your other medicines, and the side effects you want to avoid. The comparison is a starting point for questions, rather than a recommendation that a particular medicine will suit you.
Sildenafil versus tadalafil: timing and routine
Sildenafil is commonly used as needed, and a high-fat meal can delay its effect. Tadalafil has a longer window of action and may be prescribed either as needed or on a daily schedule. Its effects can last up to 36 hours, but that does not mean an erection should remain for that time. Daily and as-needed tadalafil are different prescribing plans.
Choose a routine with your prescriber rather than copying somebody else's timing. Ask what to do if a dose is missed, when another dose is permitted, and how food or alcohol may affect your specific product. A comparison can help organize questions, but it cannot calculate a safe dose for your circumstances.
Interactions and symptoms that need attention
Medicines that can interact
PDE5 inhibitors must not be combined with nitrate medicines such as nitroglycerin, or with recreational nitrites often called poppers. Dangerous falls in blood pressure can result. Riociguat and some other medicines also make a medication review essential. Tell the prescriber about treatments for chest pain, blood pressure, prostate symptoms, HIV and fungal infections.
Symptoms that need urgent help
An erection lasting more than four hours requires urgent medical attention. Sudden loss of vision or hearing also needs prompt medical assessment. If chest pain occurs during sexual activity, stop and seek emergency help; tell the responders which ED medicine you took and when. Never add another ED product to compensate for a disappointing response without discussing it with your prescriber.
You don’t need all the answers before asking for help
A clear description of what changed is a useful starting point.
Options beyond tablets
A treatment plan can include physical activity, support to stop smoking, changes to alcohol use, and attention to underlying health conditions. Counseling or sex therapy may help when worry, stress or relationship concerns are involved. These approaches can be discussed alongside medication rather than treated as competing explanations for the problem.
A clinician may also discuss vacuum erection devices, locally administered medicines or surgery when appropriate. Each option has its own practical demands and risks. If tablets have not helped, ask what the next assessment or treatment step would involve instead of repeatedly changing online products. A follow-up appointment is an opportunity to review the diagnosis as well as the treatment.
How to compare online consultations and costs
Before paying for a subscription, check what the service includes: the initial assessment, a prescription if clinically appropriate, the quantity supplied, dispensing and delivery charges, and access to follow-up. A low price per tablet may describe a large order or an introductory offer. Write down the total amount you would pay and the refill interval so different offers can be compared on the same basis.
For a US online pharmacy, check state licensing and whether a valid prescription is required. Be cautious about products marketed as prescription-strength ED treatment without an assessment. A provider link on this site is a route to information, not a promise that a prescription will be issued. Read the affiliate disclosure before following commercial links.
Make the follow-up conversation specific
If an initial treatment does not meet your expectations, describe what happened before deciding that every option has failed. Record the product, whether you followed the prescribed instructions, any side effects, and the practical issue you want help with. Timing, confidence, affordability and tolerability are different concerns; naming the difficulty helps the clinician address it. Keep the prescription or packaging available so there is no uncertainty about which medicine was used.
Agree on how to contact the service between routine appointments and which symptoms require urgent care instead. You can also ask whether involving a partner would be useful and comfortable for you. A review is a chance to revisit the overall assessment as well as the prescription. Changes to medicines or a new health problem should be discussed before continuing an old plan without checking it.
Questions for your visit
- What could be contributing to my symptoms, and which causes need investigation?
- How do my heart health and current medicines affect treatment choices?
- What should I expect from the first few attempts, and when should we reassess?
- Which symptoms need urgent help, and who should I contact between visits?
- What will the prescription, consultation and follow-up actually cost?
Common questions
Is there one best medicine for erectile dysfunction?
No single ED medicine is best for everyone. Suitability depends on health conditions, interactions, preferred timing, side effects and response. A clinician can help compare the options rather than choosing solely by brand name.
Does tadalafil work for 36 hours continuously?
Its treatment window can extend up to 36 hours after a dose used as needed. This does not mean a continuous erection. Sexual stimulation is still needed, and an erection lasting more than four hours is an emergency.
Can I combine sildenafil and tadalafil?
Do not combine ED medicines unless a clinician has specifically directed and supervised the treatment. Taking more than one can increase adverse effects and is not a safe way to troubleshoot an inadequate response.
Can stress cause erectile dysfunction?
Stress and anxiety can contribute, but physical and psychological factors can coexist. A persistent change should be assessed rather than assumed to be caused only by stress.
Do ED tablets increase sexual desire?
PDE5 inhibitors mainly help the erection response. They do not directly resolve every cause of low desire. Tell your clinician if reduced interest, pain or other sexual concerns are part of the problem.
What should I check before ordering ED treatment online?
Check the pharmacy license, prescription requirements, who provides the clinical assessment, and the full consultation and refill costs. Avoid treating a product listing as confirmation that a medicine is suitable for you.
